Forms for DIRECT DEPOSIT Reporting System
Viceroy offers forms to enhance your direct deposit reporting. Each is designed to print Statement of Earnings. Convenient perforations make it easy and efficient to separate Statements, or to use Customized Statements with our compatible security-lined envelopes. Styles available for Direct Deposit forms:
A. Non-imprinted (blank) forms
(1) Form CS091 - A stock continuous form for your impact printer. Size:
9½"W x 3½"D. Prints any Statement of Earnings, Detailed or non-Detailed,
using one, two, or three forms.
(2) Form LS093 - A stock sheet form for your non-impact printer. Size:
8½"W x 11"D, with horizontal perforations every 3½". Prints 3
non-Detailed Statements, or 1 Customized or Detailed Statement, per page.
(3) Form LS095 - A stock sheet form for your non-impact
printer. Size: 8 1/2" w x 5" d, with horizontal perforation to separate two forms per sheet.
Prints two detailed statements per page.
(4) Form CS096 (Viceroy Speedy Mailer) - A stock
continuous peel-back mailer for your impact printer. This form has an outside
file copy and a sealed self-contained envelope that will carry a Statement of
Earnings printed on the inside. The sides of the envelope are protected on two
surfaces by our Total Security lining. Size: 9½"W x 5½"D.
B. Pre-imprinted (personalized) forms
(1) Form CP091 - A continuous form imprinted with your company name,
otherwise same as in A(1) above.
(2) Form LP093 - A sheet form imprinted with your company name, otherwise
same as in A(2) above.
(3) Form CP096 (Viceroy's Speedy Mailer) - A customized
continuous form imprinted with your company information, otherwise same
as A(3) above.
