Do you need detailed Year-to-date Earning Statement or
This pay period non-detailed Earning Statement?

For detailed Earning Statement choose

CS091(B) or (C) or CS096 (for impact printer) or
LS095(B) or LS093(C) (for laser printer):

CS091- Deduction Statement for Direct Deposit, size: Continuous 9.5"x3.5"

For This Pay Period non-detailed Earning Statement choose

CS091(A) or (B) or or CS096for impact printer or
LS093(A) for laser printer.
